These records consist of State Administrative Program files, including incoming and outgoing correspondence, memoranda, and attachments; notes; budget worksheets; grant applications and review documents; public hearing notices, agenda, and presentations; work program performance schedules; applications for designation agreement; and drafts and final copies of the State Administrative Program. Dates covered are 1976-1980.

These records consist of the Work Program Progress Reports submitted between 1976 and 1981 to the U.S. Department of Health and Human Services (formerly U.S. Department of Health, Education, and Welfare), by the Bureau of State Health Planning and Resource Development of the Texas Department of Health. These reports provide detailed, activity-by-activity narrative and statistical reports of all state health planning and development functions performed by the Texas Department of Health and the Texas Health Facilities Commission. Frequency of the reports was annual, semi-annual (July 1 thru December 31 only), and quarterly. Included are originals, work papers, and cover letters and memos. Quarterly reports for 1980 and 1981 consist of Data Management Progress Reports only, in response to Condition Number 2 of the State Health Planning and Development Agency full designation agreement.

These records consist of the original copies of the proposed and the adopted rules for Data Collection (which were submitted to the Texas Register for publication), plus copies of the full comments from the public and other interested parties, staff analyses and responses. Dates covered are 1978-1980.

These records of the Texas Health Data Cooperative consist of correspondence, memoranda, mailing lists, reports, statements of accord, information forms, bylaws, minutes, presentations, etc. Dates covered are 1979-1984.

These records include the health systems plans, review manuals, data books, and proposed amendments for each of the Health Service Agencies in the state, developed between 1977 and 1981, for time-periods varying in length, extending to 1987 at the latest. Also included are two manuals for general use: the Organization and Management Resourcebook for Texas Health System Agencies, 1976; and the Statewide Health Coordinating Council Reference Guide to Health Systems Plans of Health Systems Agencies, 1981. Texas is divided into twelve Health Service Areas, which are designated as Health Systems Agencies (HSAs), each charged with developing long-range and short-range plans for the improvement of its region's health facilities and resources.

These records consist of files of applications for federal grants for non-construction programs under P.L. 93-641, P.L. 95-626, and other federal laws, which were submitted for review to the Bureau of State Health Planning and Resource Development of the Texas Department of Health, acting as the designated State Health Planning and Development Agency (SHPDA). Dates covered are 1984-1986. Eight of the 19 applications were submitted by the Texas Department of Health (TDH); the city of Houston submitted five applications; the City of San Antonio, three; the Texas Department of Mental Health and Mental Retardation (TDMHMR), one. The programs included childhood immunization, refugee health, rural health, family planning, venereal disease control, and mental retardation services. These files include copies of applications and continuation applications, staff reports, correspondence, notices of grants, etc.
