This report examines the incidence, cause, and details of U.S. friendly fire incidents during Operation Desert Storm. The purpose of this report is: (1) to furnish the U.S. Department of Defense, federal public policy makers, and the American public with a thorough explanation about the details and cause of U.S. Desert Storm friendly fire incidents; (2) to correct figures concerning the magnitude of U.S. Desert Storm friendly fire incidents, casualties, and battle damage; and (3) to recommend effective changes in technology, doctrine, and training that DOD and other federal officials should implement to help prevent U.S. military forces from firing upon fellow and allied troops in future combat situations.
